IMPORTANT LEGAL NOTICE: This document affects your legal rights. By signing, you release Forge Lacrosse Performance from liability for injuries arising from lacrosse training. Read all sections carefully.
1. Parties to This Agreement
This Liability Waiver, Assumption of Risk, Indemnification, and Release of All Claims (“Agreement”) is between:
Provider: Forge Lacrosse Performance, operated by Doug Steele, YMCA Taylors, 300 W. Lee Road, Taylors, SC 29687 (“Forge”).
Participant: The enrolled athlete and/or the parent or legal guardian signing on behalf of a minor (“Participant,” “you,” or “your”).
2. Description of Activities
This Agreement covers all programs and activities offered by Forge Lacrosse Performance, including:
Defensive Academy (Grades 7–12)
Offensive Academy (Grades 7–12)
K–6 Future Ballers youth development
Private instruction (1-on-1 training
Women’s lacrosse programs (when available)
Warm-up, conditioning, and all related activities at any Forge location
3. Inherent Risks
Lacrosse is a contact sport involving physical exertion, body contact, sticks, and hard projectiles. Participant acknowledges inherent risks including:
Sprains, strains, fractures, and dislocations
Cuts, bruises, and abrasions from sticks, balls, or player contact
Head injuries, including concussion
Eye and facial injuries
Heat exhaustion and heat stroke
Cardiovascular events during strenuous activity
Slips, trips, and falls on field or gymnasium surfaces
Injuries from defective or improperly worn equipment
4. Assumption of Risk
Participant VOLUNTARILY AND KNOWINGLY ASSUMES ALL RISKS associated with participation in the Activities, whether known or unknown, foreseen or unforeseen. Participant acknowledges that the athlete is physically capable of safe participation and will inform Forge coaches of any injury, illness, or medical condition before or during participation.
5. Release & Waiver of Liability
PLEASE READ CAREFULLY — THIS SECTION LIMITS YOUR LEGAL RIGHTS.
In consideration of being permitted to participate in the Activities, Participant hereby RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE Forge Lacrosse Performance, Doug Steele (individually), all Forge coaches and staff, YMCA of Greenville (as facility provider), and all respective officers, directors, employees, volunteers, and agents (collectively “Releasees”),
FROM ANY AND ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES arising from participation in the Activities, including claims arising from the NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by South Carolina law.
This Release does not apply to gross negligence, recklessness, or willful and wanton misconduct. This Agreement is intended to comply with South Carolina law, including Fisher v. Stevens, 355 S.C. 425 (S.C. Ct. App. 2003).
6. Indemnification & Hold Harmless
Participant agrees to INDEMNIFY AND HOLD HARMLESS the Releasees from any and all claims, damages, costs, and expenses (including reasonable attorneys’ fees) arising from Participant’s participation, any breach of this Agreement, or any act or omission by Participant causing injury to a third party.
7. Medical Authorization
In an emergency where a parent or guardian cannot be reached, Participant authorizes Forge staff to contact emergency services (911), consent to emergency treatment as deemed necessary, and administer first aid. Participant agrees to keep current emergency contact information on file with Forge.
8. USA Lacrosse Membership Requirement
All participants must maintain a current USA Lacrosse membership throughout their enrollment. USA Lacrosse membership includes accident and liability insurance benefits for covered lacrosse activities. Maintaining membership is the sole responsibility of the Participant.
9. Concussion Protocol
Any athlete suspected of sustaining a concussion will be removed from activity immediately and may not return until cleared in writing by a licensed medical professional, consistent with South Carolina state law and USA Lacrosse concussion guidelines.
10. Photography & Media Release
Participant grants Forge permission to photograph and video record athletes during training for promotional and documentation purposes. To opt out, send written notice to train@forgelacrosseperformance.com.
11. Code of Conduct
Participant agrees to treat all coaches, staff, and athletes with respect; arrive prepared; wear required protective equipment; report injuries immediately; and refrain from abusive, discriminatory, or threatening behavior. Forge reserves the right to remove any participant for conduct posing a safety risk. Conduct-based removal does not entitle Participant to a refund.
12. Governing Law & Severability
This Agreement is governed by South Carolina law. Venue for any dispute shall be Greenville County, SC. If any provision is found invalid, remaining provisions remain in full force.
13. Acknowledgment
BY SIGNING, YOU CONFIRM THAT YOU HAVE READ THIS ENTIRE AGREEMENT, UNDERSTAND ITS TERMS, AND VOLUNTARILY AGREE TO BE BOUND BY IT. PARENT OR LEGAL GUARDIAN MUST SIGN FOR ALL MINOR PARTICIPANTS.
A signed copy of this waiver must be submitted to Forge Lacrosse Performance before your athlete’s first session. Please email a signed copy to train@forgelacrosseperformance.com or bring a printed copy to your first session.
Effective: January 1, 2026 — Registration constitutes acceptance of these Terms.
1. Program Provider
These Terms govern participation in programs offered by Forge Lacrosse Performance (“Forge”), operated by Doug Steele, YMCA Taylors, 300 W. Lee Road, Taylors, SC 29687. Contact: train@forgelacrosseperformance.com | 203-376-4503.
2. Registration & Enrollment
Registration is complete only upon receipt of full payment and a signed Liability Waiver.
All participants must maintain a current USA Lacrosse membership. Forge reserves the right to deny participation to athletes without a valid membership on file.
Roster spots are limited and filled on a first-come, first-served basis upon receipt of payment.
Forge reserves the right to deny or revoke enrollment if a participant poses a safety risk or violates these Terms.
3. Fees, Payment & Refund Policy
Payment: Full payment is due at registration. Forge does not hold roster spots without payment.
Refunds:
Cancellation more than 14 days before program start: Full refund minus $25 administrative fee.
Cancellation 7–14 days before program start: 50% refund.
Cancellation fewer than 7 days before program start: No refund. Program credit may be issued at Forge’s discretion.
No-shows and mid-session withdrawals: No refund or credit.
Forge-initiated cancellation: Full refund for all affected sessions.
Make-Up Sessions: Forge does not guarantee make-up sessions for missed individual sessions. Make-ups may be offered at Forge’s discretion. Missed sessions due to participant absence are not eligible for refund.
Private Session Packs: Non-refundable once any sessions have been used. Unused sessions may be transferred to a sibling or credited toward future sessions at Forge’s discretion.
4. Session Cancellation by Forge
Forge reserves the right to cancel or reschedule sessions due to inclement weather, facility closures, coach unavailability, or insufficient enrollment. Every reasonable effort will be made to notify participants at least 2 hours in advance. Cancellations due to weather or force majeure do not entitle participants to a refund; make-up scheduling is Forge’s standard remedy.
5. Equipment Requirements
Defensive / Offensive Academy (Grades 7–12): Helmet, gloves, arm pads, shoulder pads, cleats, mouthguard, and lacrosse stick meeting current USA Lacrosse standards.
K–6 Future Ballers: Lacrosse sticks and tennis balls provided. No additional equipment required. No contact drills.
Private Sessions: Equipment appropriate to position and age as directed by coach.
Forge is not responsible for lost, stolen, or damaged equipment.
6. Code of Conduct
Participants, parents, and guardians must uphold Forge’s values: discipline, respect, accountability, and sportsmanship. Grounds for immediate removal without refund include: physical altercation or threatening behavior, verbal abuse or discriminatory conduct, use of performance-enhancing substances, deliberate destruction of equipment, or disregard for coach-directed safety protocols.
7. Health, Safety & Concussion Protocol
Participants must disclose all known medical conditions, allergies, and limitations before participation.
All Forge coaches hold current USA Lacrosse certifications including concussion awareness training.
Any athlete suspected of a concussion will be removed immediately and may not return until cleared in writing by a licensed medical professional, consistent with South Carolina law.
Forge will contact parents or guardians immediately in the event of any significant injury.
8. Photography, Video & Intellectual Property
Forge may photograph and video record training sessions for promotional use. Enrollment constitutes consent unless you opt out in writing at train@forgelacrosseperformance.com. All Forge training materials, curricula, and instructional content are proprietary. Participants may not reproduce or commercially exploit any Forge training content without written permission.
9. Governing Law & Dispute Resolution
These Terms are governed by South Carolina law. Disputes shall first be submitted to mediation in Greenville County, SC. If unresolved within 60 days, parties may pursue legal remedies in Greenville County courts.

Forge Lacrosse Performance is committed to protecting the privacy of participants, families, and website visitors. This Policy explains how we collect, use, and protect your personal information.
1. Information We Collect
Registration: Athlete’s full name, date of birth, grade, and school; parent/guardian name(s), email(s), and phone(s); emergency contact; medical conditions and allergies; USA Lacrosse membership number; payment information (processed securely).
Website: Browser type, IP address, pages visited, and information submitted via contact forms. We do not use tracking technologies that identify visitors by name without consent.
Photography & Video: If you have not opted out, we may collect photographs and video footage during training for promotional purposes.
2. How We Use Your Information
To register and enroll athletes in Forge programs
To communicate session schedules, updates, and program information
To process payments for program enrollment
To comply with legal obligations including safety and emergency protocols
To promote Forge programs through our website and social media (subject to photo opt-out)
We do NOT sell, rent, or trade your personal information to third parties for marketing purposes.
3. Children’s Privacy (COPPA)
We comply with the Children’s Online Privacy Protection Act (COPPA). We collect personal information about minor athletes only with verifiable parental consent through our registration process. Medical information for minor athletes is used solely for athlete safety and is not shared with third parties except in emergencies.
4. Payment Processing
Forge uses third-party payment processors compliant with PCI DSS. We do not store complete credit card numbers or sensitive payment data on our systems.
5. Disclosure of Your Information
We may share information only with: trusted service providers assisting in program operations; USA Lacrosse (membership verification); YMCA Taylors (facility access); emergency services (in emergencies); and as required by law.
6. Data Retention
Registration records: 3 years from last participation, or as required by law.
Medical/emergency contact info: Duration of enrollment plus 3 years.
Payment records: 7 years per financial recordkeeping requirements.
Photos and video: Indefinitely unless written removal is requested.
7. Data Security & Breach Notification
We implement reasonable safeguards to protect your information. In the event of a data breach, we will notify affected individuals as required by South Carolina law (S.C. Code § 39-1-90) and applicable federal law.
8. Your Rights
Access: Request a copy of information we hold about you or your athlete.
Correction: Request correction of inaccurate information.
Deletion: Request deletion, subject to legal retention obligations.
Marketing Opt-Out: Opt out of promotional emails via the unsubscribe link or by contacting us.
Photography Opt-Out: Written notice to train@forgelacrosseperformance.com.
9. Updates to This Policy
Material changes will be communicated by email to enrolled families and posted at forgelacrosseperformance.com with a revised effective date.
